About the Role
Reporting to the Outreach Manager, you will provide day-to-day leadership, professional support and operational oversight to a multidisciplinary team including Registered Nurses, Health Coaches, Kaiāwhina and Outreach Administrators.
You will play a key role in ensuring our outreach services are culturally responsive, community-focused, and achieve meaningful outcomes for the people we serve.
Based at one of our Auckland sites (Henderson, Blockhouse Bay or Manurewa), this role involves working closely with staff, patients, whānau, community partners and healthcare providers to ensure services are accessible, effective and aligned with The Fono's commitment to equity.
· Leading and supporting a high-performing outreach team.
· Coaching and developing staff to achieve their full potential.
· Supporting service delivery and ensuring contractual targets and KPIs are achieved.
· Strengthening systems, reporting processes and continuous improvement initiatives.
· Promoting culturally safe, patient-centred and whānau-centred practice.
· Building strong relationships with community organisations, healthcare providers and stakeholders.
· Supporting innovative approaches that improve engagement and health outcomes for Pacific and Māori communities.

· Registration as a health professional.
· Experience in quality improvement, service development or programme delivery.
· Experience using health information systems and reporting platforms.
