We are looking for a passionate and organised Outreach Administrator to join our outreach team based at Henderson, Blockhouse Bay or Manurewa.

This is an opportunity to play a meaningful role in supporting patients and whānau by helping coordinate access to healthcare services, supporting outreach programmes, and ensuring people receive the follow-up and care they need.

About the Role

As an Outreach Administrator, you will be an important part of our multidisciplinary outreach team, working alongside healthcare professionals and community teams to improve patient engagement and access to services.

You will support our patients and whānau by:

  • Coordinating patient recalls and follow-up activities for important health services, including immunisations, screening, chronic conditions and preventative care
  • Supporting outreach staff with appointment coordination, scheduling and community-based activities
  • Identifying patients who may need additional support and connecting them with the right services
  • Maintaining accurate records, databases and reporting information
  • Supporting patients who may face barriers to accessing healthcare
  • Working alongside nurses, social workers, health coaches, kaiāwhina and community partners to provide coordinated support

Your work will help ensure patients do not miss important opportunities for care and are connected with the right support at the right time.

What You Will Bring

We are looking for someone who brings a combination of strong administration skills and a genuine passion for supporting Pacific communities.

You will ideally have:

  • Previous administration experience, preferably within healthcare, community services or a similar environment
  • Strong organisational skills and attention to detail
  • Excellent communication skills and the ability to build positive relationships with patients, whānau and colleagues
  • Experience working with Pacific, Māori or diverse communities
  • Confidence using systems, databases and Microsoft Office applications
  • The ability to manage multiple priorities and follow through on commitments
  • A commitment to improving health equity and supporting better outcomes for underserved communities
  • Experience with patient management systems such as Medtech, primary healthcare environments, outreach services or community programmes would be an advantage.
  • Pacific language skills and an understanding of Pacific models of care are highly valued.
