Support Worker Position Description
Summary:
· provides care and support for Tāngata whaiora (‘residents’) with complex mental health needs.
Skills required:
· observes and reports on residents’ mental health status.
· helps residents to manage and understand the symptoms of their psychiatric illness.
· identifies and reports risk.
· helps with self-care and other daily activities.
· Understands and recognises early warning signs which may lead to a deterioration in health.
· Engages meaningfully and works in partnership with our residents.
· Focuses on individual strengths to support recovery.
· Contributes to whānau ora for Māori.
· Encourages and supports family/whānau to participate in recovery.
· Ensures that families/whānau have access to information, education, and support.
· Recognises that residents and their whānau are part of a wider community.
· Challenges stigma and discrimination.
· Promotes shared values.
· Complies with legislation, regulations, standards, codes, and policies relevant to their role.
· Actively reflects on their work and practice and works in ways that enhance the wider team to support recovery.
Tasks
· Works with Tāngata whaiora and their whānau to identify needs/pathways to recovery.
· Considers the individual’s family/whānau and history of mental health.
· Observes and reports on any changes in mental health status.
· Supports residents to manage agreed areas of their lifestyle to improve their overall health.
· Jointly plans recovery with residents, family/whānau and the wider care team.
· Gives residents advice and support, focusing on recovery/rehabilitation
· Administers prescribed medication according to medication policy guidelines.
· Assists with managing risk by following all Health and Safety policies.
· Helps residents with self-care and other daily activities.
· Supports residents to engage with their community.
· Supports residents, and whānau to meet the agreed goals in recovery plans.
· Facilitates and coordinates appointments.
· Educates residents and their caregivers about early warning signs.
Responsibilities
· Works within a team of support workers to support residents.
· Works within the guidelines of the Mental Health Act 1983.
· Documents and shares information.
· Reports any concerns or potential risk.
Must have:
-Full NZ Drivers Licence
-Proof or right to work in NZ
-Good spoken and written English
-Familiar and capable with Office 365 and data entry
-A Waiheke Island permanent address
Desirable:
NZ Certificate in Health and Wellbeing L4 Mental Health and Addiction
