The Industry

Primary Health Organisations known as PHO’s provide health services either directly or through it’s contracted providers. The aim is to provide and maintain the health of the enrolled PHO population, by ensuring that general practice services are connected with other health services to ensure a seamless continuum of care.

The Organisation

Marlborough Primary Health (MPH) promotes and supports community health services by leading and coordinating a range of health care services to an enrolled population of over 47,200 people. It achieves this by contracting services to General Practices, Marlborough Urgent Care, and other Primary Health providers such as Māori and Pacific Providers.

The Job

Reporting to the Community & Operations Manager. 

Key aspects of the role include:

  • Client base is predominantly of Spanish-speaking language.
  • Clients may receive the service on an individual basis or in a group setting.
  • Case management.
  • Strong relationships with other organisations, in particularly Red Cross and General Practices.
  • Have strong cultural awareness to support our diverse community. 
  • Be a resource person to the wider MPH team and general practice teams that supports patients needs.
  • Evaluates the individuals progress and treatment against identified goals.
  • Provides advice to the multi-disciplinary team regarding therapeutic approaches, resources, services, and agencies.

The Ideal Candidate:

  • Is Spanish speaking.
  • Have current annual practicing certification.
  • A relevant tertiary level qualification (e.g. social work, health science) and/or relevant experience working with former refugees or similar group (e.g. low income, Māori, Pasifika, migrant).
  • Has experience working in primary care, or community settings.
  • Awareness of stepped models of care.
  • Is self-motivated, can work autonomously and can enjoy working as part of a team dedicated to improving peoples’ health and wellbeing.
  • You must have an understanding and sensitivity to working with all cultures.
